net/cxgbe: fix duplicate MAC addresses in MPS TCAM

During MAC address insertion to MPS TCAM, add a default mask when
the mask is not explicitly specified. Othewise, driver misses the
mask comparison and ends up inserting duplicate entries in the
MPS TCAM.
